INDIVIDUAL HEALTH INSURANCE

How do I apply during the Annual Open Enrollment Period for an Individual Affordable Care Act (marketplace) health insurance plan?

The annual Open Enrollment Period runs from November 1, 2018 through December 15, 2018. This is the only timeframe provided to apply for a 2019 ACA health insurance plan effective 01/01/2019.

Outside open enrollment, you can only enroll if you qualify for a Special Enrollment Period (SEP), such as having a baby, getting married, losing employer based coverage, etc.  Please contact our office during the interim to see if you qualify for a SEP.

In additional to ACA comprehensive health insurance plans, we offer the following coverages below with the provider insurance carriers. * Please note, there may be additional companies that we offer that are not listed below.

Temporary (short term) Health Insurance

International Medical Insurance

Individual Dental and Vision Insurance

Accident, Critical Illness, Doctor, and Hospital Indemnity Insurance Plans

If you wish to obtain more information on these services or request a quote,  please contact our office and we will gladly assist you with your needs.

*IMPORTANT: If you received subsidy for your current plan for the 2018 plan year, please note that you will need to re-apply for the 2019 plan year, or you will risk losing any subsidy that may apply.

Below are step-by-step instructions that will help you through the process of:

  • Determining if you are eligible for a subsidy (tax credit) from the government to help pay your premium and/or out-of-pocket health care costs.
  • Finding a plan that fits your needs and budget.

Step 1: Choose the carrier link and click.

Step 2: Enter in your basic information along with any enrolling dependents.  This will include your zip code, date of birth, gender and smoking status.

Step 3:  It will ask if you want to find out if you qualify for a premium government subsidy.

  • If you DO NOT wish to apply for premium subsidy, please select “No” and proceed to shop for plans.
  • If you would like to find out if you qualify for government subsidy, please contact our office directly for further assistance.  You may also go to www.healthcare.gov to determine eligibility for subsidy and to apply for subsidized health insurance plans. However, please make sure to provide the information in step # 8 below, when prompted.

Step 4:  If you selected “No,” you will be directed to the next page listing the plans available.  You will have the option of choosing the metallic level of coverage that best fits what you need.  You will be able to choose from platinum, gold, silver, and bronze, where each level represents a different percentage of coverage.  You will also have your choice of HMO, PPO and High Deductible HSA plans and hospital/physician networks.

Step 5: Once you have selected you medical plan, you will be directed to the next page, which includes the dental coverage options available.  You will also have the opportunity to use the Check Provider Finder tool to verify that your doctor is in the network selected.  After making the appropriate selections, click “Continue” to proceed with the enrollment process.

Step 6: Now you will be prompted to log in to your account or create a user name and password for the Blue Cross Blue Shield of Illinois website.  If you created a log in last year, please use the same information to sign in.  You will use this login each year, so keep it in a safe place.  Do not share your user name and password with anyone.

Step 7: After you have logged in, you will be guided through the final steps to the enrollment process, which includes accepting the terms of use, applicant information, and payment information.  Once you have successfully enrolled in your selected coverage, make sure to print out the confirmation page for your records.  You are all set now!

Step 8: If the site prompts “if anyone was assisting you with this application,” please include the information below to ensure we are the servicing agent on your policy:

                        Type: Agent/Broker

Agent: David Marcus

Agency: David A Marcus & Associates, Inc.

Frequently Asked Questions:

If I do not want to apply online, how can I apply?

Please contact our office to obtain a paper application

What is Pediatric Dental? Do I have to elect it or can I waive it?

This is an Essential Health Benefit that must be covered under Affordable Care Act health plans for children under the age of 19.  Some carriers have this benefit embedded into their medical benefits, while others have the option to waive it.  If you choose to waive this benefit without having compliant Pediatric Dental through another plan you will be charged a penalty.
